I to the best of my ability, have been honest and forth coming in any and all medical questions regarding my personal circumstances. I am responsible for any misleading or false answers that may affect my training or health advice. I am aware of the risks in observing or participating in the activities offered and sponsored by Matter Athletica and I understand that all sports or fitness that I will execute and participate in are entirely at my own risk and perils. I assume complete responsibility and liability for those risks and for the injuries that may occur as a result of these risks, even if injuries occur in a manner that is not foreseeable at the time, I sign this agreement. I realize that by voluntarily assuming the risks involved, I will be solely responsible for any loss or damage I sustain, including personal injuries to me, damage to my property, or damage arising out of my death. Matter Athletica assumes no responsibility for any health implications associated with practises and I am aware everything that is spoken about is to be kept private. Matter Athletica is not a medical professional, and everything is for educational purposes only
In signing this I understand, results in a given time frame are not promised or guaranteed. I will execute plans to the best of my abilities. I recognise that at least 12 weeks is advised for participation and work with Matter Athletica. I guarantee a 2-week grace period before finalizing any services with Matter Athletica. I recognise a change of mind is not guaranteed for refund of paid services and that medical documentation may be required to holt future services.

I understand follow up emails may reach out to me to check in and see how I am progressing. I agree to future communication from matter athletica in follow ups or timely check ins.
